- Names of all attendees (even if they aren’t eating food)
- Ages of all youth attending under 18 years old
- If anyone has CPR certification, please indicate who is CPR Certified
- If you are willing to drive the trailer from Johns Creek to the campsite or back to Johns Creek from camp, please indicate that.
- Indicate you have read and agree to the Behavior Expectations for all attendees
